Practices across the UK have been informed that flu vaccine deliveries from the supplier Seqirus are going to be delayed by at least one. A full course is currently two doses of any approved vaccine. At Haden Vale Medical Practice, we will continue to wear protective face coverings and still observe social distancing.
In return we advise that a protective face covering is still expected to be worn by all before entering the surgery foyer and whilst in the building. Anyone that is exempt will be encouraged to wait outside the building or stay in the car until their appointment time. Thank you for your co-operation in keeping us all safe.

It also includes your name, address, date of birth and unique NHS Number which helps to identify you correctly.
