If possible also add your NHS number in the form. If you do not know your NHS number then please contact your previous surgery and they will be able to give you this over the phone. A few patients living outside this boundary, and who have been with the practice for some time, are still registered with Lime Grove.

I work in the NHS in direct contact with patients. I know how to treat patients.
Unfortunately I find the receptionists at this surgery to be lightly rude and seem to have no compassion when you're unwell and in need of an aptmt. They don't seem to have any people skills at all.
I used to be registered at Crich surgery and all the staff there were 5/5. I was devasted to have to leave when I moved.

I have been a patient at Lime Grove Medical centre since I moved to the area a year ago. I have been thoroughly impressed each time I have had need to use the surgery.
The reception staff are friendly and helpful. They go out of their way to try and help find a solution even when they are obviously under time pressures and demands of multiple patients.
I have seen several GPs over the last 12 months and all have been very professional. On each occasion I felt listened to, was appropriately examined and had the opportunity to decide the best way forward. I have only seen a nurse one time but was
