My Care Record facilitates the lawful sharing between health and care professionals through access to records by participating partners. The data is being processed for the purpose of delivery of a programme, sponsored by NHS Digital, to monitor urine for indications of chronic kidney disease (CKD) which is recommended to be undertaken annually for patients at risk of chronic kidney disease e.g., patients living with diabetes.

Services
The Out of Hours Service will take over from 6.30pm to 8.00am Monday to Friday and 6.30pm on Friday to 8.00am Monday. Please do not ask to see a doctor out of hours unless you genuinely cannot wait until the surgery re-opens. An enlarged printed version of the patient leaflet is available to those patients who require it.
When a doctor wishes you to have regular medication, you will need a Medication Review every 6 months. Failing to attend your review may cause a delay to your request. Please understand our cautionary approach on this matter as uncontrolled issues of repeat prescriptions could, under certain circumstances, be potentially dangerous and compromise your health.

Storing information in one place makes it easier for healthcare staff to treat you in an emergency, or when your GP practice is closed. This information could make a difference to how a doctor decides to care for you, for example which medicines they choose to prescribe for you. No, it is not compulsory.
It is important to make this initial appointment as early as possible - at least 6 weeks before you travel - as a second appointment will be required with the practice nurse to actually receive the vaccinations. These vaccines have to be ordered as they are not a stock vaccine. Your second appointment needs to be at least 2 weeks before you travel to allow the vaccines to work.
