The deceased's own doctor or hospital doctor will issue the Medical Cause of Death Certificate to the family or next of kin. You will need this to register the death within five calendar days. If this is the case, the Coroner will become involved. The police attend on the Coroner's behalf. A nominated funeral director will remove the deceased to hospital for further investigation.
Sadly, when someone dies in a hospital or hospice it is often expected. The doctors issue a Medical Cause of Death Certificate. They may hand this directly to the next of kin or pass it to the establishment's bereavement officer. The next of kin can use this certificate to register the death. Once this certificate has been issued and we have the "Green" form we can come and take the deceased into our care.
If someone dies in a nursing home, and the death was expected, a registered nurse at that home will confirm the death. If they die in a care home, and the death was expected, a competent adult can confirm the death. Please contact us. The deceased's own doctor will issue the Medical Cause of Death Certificate to the family or next of kin.
You need the Death Certificate issued from the deceased's usual doctor. You then need to make an appointment with the registrar. For North Yorkshire County Council, you can do this online at Registering a death or call 01609 780 780. During evenings and weekends please call 01609 798 832. We suggest you ask for a minimum of six copies of the original death certificate to help you with estate matters later.
A coroner will automatically be involved if the person who died was placed under a DOLS Act (Deprivation of Liberty Order). The UK coroner will send a form to the registrar informing them that the death can now be registered. The coroner's office will tell you if they need a medical certificate of cause of death from the doctor.
