To make things easier for you to register, download a New Patient Registration Form and New Patient Questionnaire. Complete these at your leisure, print them off and bring them into the practice. It is practice policy to ask for the completed forms to be accompanied by photographic ID and proof of your address.

Our Nursing team runs multiple clinics each week. The practice has a dedicated Mental Health Triage Nurse working with the practice team offering patient appointments on a Tuesday each week. Inhalers need to be safely disposed of to prevent propellant gases which are powerful greenhouse gases escaping into the atmosphere.
Services
Bank Street Medical Group aims to ensure the highest standard of medical care of our patients. To do this we keep records about you, your health and the care we have provided or plan to provide to you. Our Data Protection Privacy Notice provides details of the collection and use of personal information and you can find out more in the leaflet What is GDPR.
If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice. Thereafter you will need to register as a temporary or permanent patient. You can be registered as a temporary patient for up to three months.
By telephone on 01334 651201 (we do get cancellations at short notice, so it is always worth checking). Please note: we are unable to process appointment requests received via email or online forms. For medical advice when the surgery is closed telephone NHS 24 on 111. Call 999 in a medical emergency - when someone is seriously ill or injured and their life is at risk.
There are two types of prescriptions that will be on your medical record if you are prescribed medication from your GP. Acute: This is medication that is prescribed to you for a short period of time ie antibiotic, etc. It may also be medication that you are being started on for a condition to see how effective it is.
Cervical smears help prevent cancer of the neck of the womb. Cervical cancer is the most common cancer in women aged 25 to 35. A smear is the best way to protect yourself from the disease. Your smear test can pick up changes to your cells which, if left untreated, could turn into cancer. All women in Scotland aged 25 to 49 are invited for a smear test every 3 years, while women aged 50 to 64 are invited every 5 years.

Receptionist is so very rude and clearly thinks she's a doctor. Explained I need antibiotics but her attitude was disgusting she really didn't care and told me she would put me in for an advice call but she doesn't know how that would be any good. Turns out I got my antibiotics and she expected me to go till middle next week for them. Maybe a refresher course on what her job actually is and how to have some manners.

I've been with this practice since I moved to Cupar in 1972. Obviously personnel have changed over the years - when we came here there were only two partners in the practice and no health centre - but throughout we have had helpful and expert service, for which we are grateful.

The young lady that works in the reception needs a new job or just maybe her manager needs to closely monitor her very ugly bad attitude. Disgusting front of house service and uneducated beyond anyone’s standards not to have the nerve to reply after insulting me! A simple sorry from the young lady would have stopped me from CLEVERLY writing this review. I felt so sorry for the doctor as they had to take the wrath from me when it should have been the young lady in question. Once again this review is not on the Doctor’s professional service, it was on the person you have to deal with before you

Always get an appointment when I need one. Staff, nurses and doctors are efficient, professional and kind. I'm glad that they are my doctors.

This is a poor excuse for a medical practice. This is for me and my partner. Poor palliative care for a relative, can never get prescriptions or appointments, discrimination regarding another relative, all staff are generally rude and don't seem to understand the concept of duty of care or in fact they are a public service with prime purpose to serve the local community and manage family medical care. Have been told to go private and pay for own health care despite the fact everyone already does through their taxes which is then awarded to this practice in funding for their services which in turn

Where to start?! You need to ring about ten times before they will answer their phone. poor communication between themselves and local pharmacies - always messes up your prescriptions!
